🔎 What is Arizona Form 285?

If you're working with Gbooks to fix a tax issue with the Arizona Department of Revenue (AZDOR), we’ll need you to sign a short authorization form called Arizona Form 285. This form gives our team permission to talk to AZDOR on your behalf and get access to your state tax records so we can help resolve your case.

Without this signed form, AZDOR won’t let us speak with them—even if we prepared your tax return.


🤝 What Does the Form Allow Us to Do?

Once it’s submitted and accepted by the state, Arizona Form 285 allows us to:

  • 📞 Speak directly to AZDOR about your tax issue
  • 📄 Request documents or transcripts from your account
  • 🧾 Discuss payment plans, penalties, and balances due
  • 📬 Respond to notices or letters on your behalf

This form does not give us access to your bank account or financial control—it’s just the minimum authorization AZDOR needs to work with us on your file.


📩 How to Get the Form

Please reach out to your Gbooks relationship manager and ask them to send you the Arizona Form 285. We’ll fill in our representative section and send you the correct version to sign.

⏱️ What Happens Next?

As soon as we receive your signed Form 285, we will submit it to AZDOR. It takes 5 to 10 business days for the state to process this form and update their system.

AZDOR will not discuss your case with us until that processing period is complete, so we appreciate your patience while we wait for access.
